The Breast Care Navigator (BCN) assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes for Mammography patients from diagnosis back to wellness.
Responsibilities Coordinates and assist patient's access beginning with Mammography tests to treatments if necessary. Assist patients and addresses barriers to access; assist patients with test and treatment schedules. Explain procedures and schedules office visits to ensure minimal patient waiting time and anxiety.
Assesses the medical, physical, and emotional status of patients and designs and implements a plan of care to meet each patient's individual needs, based on diagnosis. Develops individual treatment strategies to address and resolve unique situations presented by patients.
Provides guidance and counseling to patients. Works with patient in dealing with the diagnosis of a positive biopsy and advises on the next steps to treatment.
Collaborates with PCP or breast surgeon to decide the next steps.
Will facilitates the continuity of each patient's care through collaboration and referral with other health care professionals to support the patient.
Minimum of Associates Degree in health or health-related field required. Registered RN with experience with breast cancer patients and the process of certification for Breast Imaging Center of Excellence (BICOE) preferred.
2. Must be self-directed and able to organize and manage multiple tasks/projects simultaneously; ability to promote programs and services to community, ability to provide a caring nurturing environment for patients ; Extensive understanding of Mammography and breast cancer patient care; proficient in review of medical reports and other patient care information; demonstrated skills in verbal and written communication; ability to maintain strict confidentiality; ability to promote and build teamwork and multidisciplinary care concept; able to establish working relationships with ordering physicians to coordinate comprehensive care for patients.
3. Three of more years of patient experience, preferable with breast cancer patients.
